Sports Medicine Forms/Portal Directions

Step 1

Click on the ‘Portal’ link.

Step 2

Complete prompts as directed:

  • Athlete ID: new
  • Password: new
  • Database: atsuwplatt (this domain should already be filled and unable to change)

Step 3

Once you select “Login,” a popup will appear. Select “Consent & Allow Cookies.”

Step 4

This will lead you to the online documents. The first tab is general information about sport, name, phone number, address, etc. On this page you will need to choose an Athlete ID.

Please use your email username.

  • Ex. John Smith -> smithj@uwplatt.edu use smithj as your username

A password will also be needed. Choose something you will remember, as this will be the method of turning in documents for each subsequent year.

Copies of your insurance card must be uploaded onto the Insurance page.

If your insurance is ongoing, select June 1st of the following year as the end date.

  • Ex. Start date: 01/01/2021 End date: Ongoing -> 06/01/2022

Step 5

Additional documentation must be downloaded through the final tab, eFiles.

  • The UWP Physical, completed by your Primary Care Provider, and the UWP Insurance forms must be submitted by all student-athletes.
  • The UWP Sickle Cell form must be completed by all freshman, transfers, and new student-athletes.
  • The UWP AHDH/ADD form need only be completed by student-athletes who take medication. All additional required ADHD/ADD paperwork must be attached and submitted with the cover page.

Step 6

Return to one of the other tabs, scroll to the bottom and click 'Save Athlete Info'. This will bring you to a new page with more tabs. Verify the information you have submitted in the following tabs.

Step 7

Select the eFiles tab. Here you will be able to upload the documents you downloaded on the previous eFiles tab. As stated before, these must be submitted:

  • The UWP Physical, completed by your Primary Care Provider, must be submitted by all student-athletes.
